•   Sum­ma­ry of your ill­ness writ­ten by your refer­ring doctor
•   Pho­to­copy of your iden­ti­ty card or passport
•   Manda­to­ry: Recent med­ical report sum­ma­riz­ing the his­to­ry of the dis­ease, the gen­er­al con­di­tion of the patient when address­ing the file to the Foch Hos­pi­tal, past treat­ments and treat­ments cur­rent­ly received and any oth­er rel­e­vant information
•   If applic­a­ble: oper­a­tive med­ical report, report of hos­pi­tal­iza­tion, his­to­log­i­cal report, report of med­ical imaging
•   Recent bio­log­i­cal and patho­log­i­cal findings
•   This ques­tion­naire com­plet­ed by yourself

In case of accep­tance of your file, a quo­ta­tion will be sent to the enti­ty cov­er­ing your expenses.

The lat­ter should then pro­vide the Foch Hos­pi­tal with a finan­cial guar­an­tee cov­er­ing all the costs indi­cat­ed in the quotation.

If you are cov­ered by the health insur­ance of your coun­try, please pro­vide the Euro­pean “Rights to sched­uled care” S2 form pro­vid­ed by your health insur­ance, and the signed quote.

If you are a Euro­pean cit­i­zen but you are not cov­ered by a health insur­ance, please fol­low the pro­ce­dure described in the first cat­e­go­ry “If you do not have any health insur­ance.”.
